I AUTHORIZE DeKalb Community Bank to post the payment transactions generated from the Bill Paying Service to the account indicated on the Application. I understand that I am in full control of my account, and that my 100% satisfaction is unconditionally guaranteed. If at any time I decide to discontinue service I will provide written notification to DeKalb Community Bank. My use of the Bank@Home Bill Payment Services signifies that I have read and accepted all the terms and conditions of the Bank@Home Bill Payment Services.

I UNDERSTAND that payments may take up to 10 days to reach the vendor and that they will be sent either electronically or by check. DeKalb Community Bank is not liable for any service fees or late charges levied against me. I also understand that I am responsible for any loss or penalty that I may incur due to a lack of sufficient funds, (NSF fee $28.00), or other conditions that may prevent the withdrawal of funds from my account.

I choose to pay my bills through the Internet. I understand that I’ll pay $7.50 per month for the first fifteen transactions and .45¢ for each transaction thereafter.
